FOR MEDICARE APPOINTMENTS – PLEASE COMPLETE BOTH OF THESE FORMS BEFORE THE APPOINTMENT
So that we can be better prepared for our appointment, please complete the TWO STEPS below. This will help the process to be as efficient and effective as possible.
STEP 1: Please complete this scope/permission form. We are required to document your permission to discuss Medicare options. This does not change anything that you are currently enrolled in, nor does it obligate you to do anything. It merely gives us permission to have this discussion. IMPORTANT: All Medicare Beneficiaries must complete a permission form (e.g. spouses each need their own even if scheduling a joint appointment).
STEP 2: Please complete this Medicare pre-enrollment form. This will help us to be better prepared for the appointment.
